We are happy to file insurance as a courtesy to our patients if we are provided with accurate and current insurance information. You will be expected to pay any amount that is determined not payable by your insurance plan, such as deductibles and percentages. The amount not covered by your insurance is due at the time of service, including deductibles and co-payments. You are responsible for payment regardless of any insurance company’s arbitrary determination of fees. However, if we do not receive payment from your insurance company within 35 days after the submission of a claim, you will expected to pay any balance on the account in full within 10 days of notification.

We must emphasize that as healthcare providers, our relationship is with you, not your insurance company. Your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract. Please be aware that some services provided may be non-covered services by your insurance company.
